GP for me
The Victoria Division of Family Practice (VDFP) has chosen the Esquimalt MFRC to be part of their pilot project A GP for Me.
The VDFP is a non-profit community based group of family doctors working together to help improve access to local primary care. To support this goal, the VDFP has launched a pilot project that will assist patients who are without a family doctor (also called a GP) to find one. Only 25 families can register to this pilot project through the MFRC.
To be eligible to participate in this pilot project, you must:·
- Not currently have a family doctor;
- Live in the VDFP catchment area, which consists of Victoria, Esquimalt, Oak Bay, Saanich and View Royal – the Westshore residents unfortunately cannot participate yet to this project;
- Have a valid B.C. care card; and
- Fill in a Request to Participate form at the CPAC MFRC location (2610 Rosebank Rd), which will then be mailed to the VDFP.
If you wish to apply, please respect the following guidelines:
- Go to the CPAC MFRC location (2610 Rosebank Rd, Colwood) during the opening hours of the MFRC and request to register to the A GP for Me pilot projet.
- Fill out the Request to Participate form (one form per family).
- The MFRC Staff will make you a copy of that form, as you will require the Request Number indicated at the bottom of the form when the coordinator of the pilot project contacts you.
- Requests to participate are processed on a first-come, first served basis and participants must live in the designated areas (Victoria, Esquimalt, Oak Bay and View Royal only).
Submitting this request does not guarantee that a match will be made to a GP, as there may not always be a GP available who can take new patients. The Esquimalt MFRC takes no responsibility in the outcome where a match is not possible between the families and a GP.
This project ends May 31st, 2016.
